Selwyn Village Healthcare is closed for Christmas – repeat prescription requests won’t be processed until we reopen on 5 January

If you do not have a patient portal account, you can sign up in-person at our reception. Photo ID and a personal email address are needed

Prescribing policy:

Our prescribing policy is based the Medical Council of New Zealand’s Good Prescribing Practice guidance.

  • Controlled/addictive medicines will not be started / increased for any new patients
  • All prescriptions & repeats are at the discretion of the prescriber
  • Medication review requires a full standard appointment for safety reasons – please don’t expect to cover other issues
  • Medication review appointments are always required (repeats not offered) for:
    • Medications that are new or recently changed, prescribed elsewhere, controlled or addictive medicines
    • Patients with two or more complex health conditions
    • At the prescriber’s request
  • Review appointments may be done 6-monthly (eg. alternate repeats), for other stable situations if the prescriber agrees. This is the maximum for the majority of medications
  • A small number of low risk medications / conditions can at times have review appointments done 12-monthly. No prescriptions can be done for patients that have not been seen in the last 12 months

Script repeat routine (3 working days) $28
– Children 0-13 years: Fees for routine requests are currently discounted to $0

Script repeat urgent (next working day) $38
– Children 0-13 years: Fees for Urgent requests are currently discounted to $13

While we are shut over public holidays we are unable to process requests until we reopen

Medications

Please list a new line for each medication you would like prescribed, including your current dosage and when / how often you take it. If this is unclear then your prescription may be incorrect or unable to be completed.

Please comment on any usual medications you don’t need this time, home BP recordings, or any other information about your prescription request:

  • Medications are not new or recently changed
  • They have had a medication review appointment at Selwyn Village Healthcare within the last 3-12 months
  • Underlying health conditions are stable and well controlled. Patients with multiple and/or complex conditions usually should be seen for each prescription
  • I have no concerns about side effects or medication safety / effectiveness
  • I have not been asked to book an appointment for medication review
  • I have enough supply to last me in case I’m asked to book an appointment
